How to create custom alert popup screen or block?

Hi Folks,
Greetings!

Kindly suggest me what is the best option for my below query 

In Web application:

On logout button click I would like to show popup with text "Do you want to logout?" and two "Yes" and "No" buttons. I should get the action is taken on popup screen "Yes" or "No" button into parent screen from where Popup screen called so that I could take next action.

From what I understood it seems that it would be better to use a modal showing these 2 buttons.
The logout button would only do the toggle.

Actually I was trying Modal since long time  but not able to get that look and feel what is shown in doc, may be I am not able to provide Modal required things anyway as per doc I was gone but did not get same look. From doc I am not getting completely how to use Modal. Have you any other doc for which has complete steps to implement Modal or any other tool like Modal. :(

Yes, action is ajax submit. Whatever steps mentioned in doc I was gone through that but I don't know where I am lagging. 

Did you saw that use case, did it not work for you?
I did a quick test in my personal area and was working fine.
