On adding new record, old record is getting deleted

In the below Oml file, I'm facing several issue

1. if  I add new school, the previous added record is deleted

2. Same with the stationary, even after selecting multiple options from Select Stationary dropdown tags I can only see one option in database. 

POP Over Menu

3. Edit option in Home page is not fetching the Stationary data which I added.

4. Unable to delete the record using the delete option in home page.
