i have create the custom app for doctors and patient and need to know how doctors and patients will be considered as a user to login or any restriction or limit to login?

That is up to you. So basically, you can filter what people/users with a certain role can see or not. After the login, you check the user role and you can redirect users to different pages, allowing users to see more or less information, but that is up to your business requirements.

If you are using a personal environment yes, you have a limited amount of active users you can have.
