To achieve this, you need a platform that brings together multiple disparate data points and systems, until then, you won’t have a true picture of the full costs of the patient care chain. Secondly, providers need to be able to ingest consumer data from sources outside of the four walls of the hospital or doctor’s office to have an accurate picture of the issues and lifestyle of the patient. To do this, your systems need to have an open API layer to plug into 3rd party apps and mHealth devices. Thirdly, you’ll need to make sense of all of this data, building a “patient profile” using advanced analytics and predictive patterns will be critical to not only value-based care but fundamentally driving down the total cost of healthcare per patient.
