Congratulations on the component.

I have a few observations & suggestions.

1. It would be better to have label position configurable for each control, which overrides the form level setting for that control.
2. If the control type is text area, the rounded corner CSS is missing.
3. Yes or No control type is confusing ... suggest to make it as a checkbox. 
4. There is no option for implementing a radio button.
