[LiveCamera] LiveCamera Forge Component - Error on Administration Page

I am trying out LiveCamera forge component but facing problems.

I have below error message on Administration page and can not save the settings.

On New Stream page, have below error upon clicking on Test button.

Any advice would be greatly appreciated.
