I am not build yet but you can start like to design DB first. Create entity like User, Medication and Reminder .Create relation between them.
Define App feature like User Registration/Login,Pill Reminder Setup ,Notifications and History Tracking and many more as per your requirements.

Design UI like Home Screen ,Medication Screen  or Reminder Setting etc.
