What should be the Solution to Form Source record ?

A Form needs a Source Record, otherwise you'll get the error you mentioned. This, however, is very, very basic OutSystems, so I have the feeling you didn't take any of the beginners courses? If that's indeed the case, please click the "Learn" tab on the top of the page, and start learning.

Hello i advise you to check this guided path https://www.outsystems.com/learn/courses/114/developing-web-apps-outsystems-11/ very usefull and give a very detailed explanation of outsystems basics.
On the other not i dk if ou will be able to do something with this information but you can create an empty variable of record type and place it in the form to remove that error. With this you will lose all the validations and ability to arrange the structure a form can provide but it will give no error.

Honestly, someone asks a simple question and you point them to a 24 hour course. By all means point them to the course as a guideline, but answer the question.
This kind of lack of answering a question to leads to consternation and a developer simple saying "screw it", and not progressing.
If a user comes here with a question you know the answer to, tell them, and then question them on why they don't know the answer themselves if it is in the training.
